When you say stuck, do you mean the adapter release is not working or it is and the pad with not let loose?
The adapter release pressed in, but the pad would not move. I ended up having to cut the plastic disc in half to release the adapter, which isn't very satisfactory.
I had this happen when one of the small balls popped out of adapter and did not notice. I took a grip on it with two hands and wife held down the release until it popped off. I thought it was going to snap.

Its all there. I did hit the adapter with some lubricant afterward. I haven't tried using it since.
I've had an Edge pad be on the adapter rather firmly, but getting my hands under it just right usually broke it loose. Your situation seems a bit on the extreme side of things.

They don't make the good qr adapter for the DA, only the rotary. Or they did when I looked last.
I had that happen, one of the small retention balls came out and I didnt notice and the pad must have spun on the adapdter and the plastic actually melted to the adapter.
