I am new to polishing and sure I started with some bad techniques, but the LC 6.5" pads I have used for well over a year on 5 cars are all in great shape except for some staining.
All on a flex 3401.
Not sure if this relates to your issues and I do think the hooks of your Velcro on your machine are too big/loose/sloppy to solidly connect to the pads, but I noticed something recently on my GG 3" polisher.
I lifted the 3" polisher up off the car surface while it was still spinning pretty fast and it caused the pad to deform a bit as it spun around trying to fly off the polisher.
Nothing tore but it can't be good for the pad.
Do you lift the polisher off the surface while it's still spinning a lot?

No, otherwise it would fling product everywhere. I keep it touching until it stops.
