Hi Guys,
This is used as decoration, at my favorite pizza joint, they do not know, nor the persons selling to them; what the make & model truck this door came from. They had a local artist, do the state symbols Bear for CA, Longhorn for TX. Owners migrated to TX from San Jose CA.
The door says "Custom Cab" perhaps A Ford, but not sure from there:

Here is a photo of a 1960 Ford Custom Cab. Chevy also had a Custom Cab. The door styles are similar. However, Ford placed the words Custom Cab on the door, just below the window. Chevy placed the words on the fender and were included in a larger trim piece.

Wow! I found this at ebay, looks real close, seller (can't believe the price) only narrows it down to btwn 1957 - 60. But with my poor eyesight looks allot like your pic..... If I'm seeing cockeyed, let me know:
FYI - That's one pretty truck, luv to own that one!
