My 3401 fell off the toolbox and broke the tabs on the D-handle.
Where would be the best place to get a replacement ?

By going to the replacement parts site for the 3401, you'll see there is a D-handle available: part# 333.565 @$10.85

Note:
It looks like the D-handle of the 3403 could be compatible(?)

I did a search and found a thread you give this same information on.
Just ordered a handle for the Flex and also a new cord for my old Makita 9207s rotary from Powerhouse Distributing .
